Amendments to assigned visa

Hi All,

I have a few questions regarding my visa. Will be a real help if I can get some help.

1 ) My employer accidentally made a typo in my family name. Does this mean she has to cancel the COS ? If so, do I have to wait for the next cycle to get a new COS ?
2 ) The duration for which the COS has been requested is 4 years. I am assuming I should be applying for a tier 2 general - 3 years ? Can the end date of an assigned COS be changed ?
3 ) Finally, my employer said "N" to the question of is there a need to travel in and out of UK ? Does this mean I can't leave UK for a vacation ?

Please let me know as I am a little worried about Q1. Any immediate help is greatly appreciated.
