Documents for applying for PIO Card for UK born minor child

Hi,

I know the question seems to be lurking in many topics and posts around here, but I am not able to identify correctly what should be the list of documents that I should provide for applying the PIO card for my 8 months UK born daughter with British passport (acquired via MN1) to parents both having Indian passport and on ILR.

P.S. My daughter has neither applied for indian passport nor issued an Indian birth registration certificate.

She's got only her birth certificate from council, MN1 certificate issued by UKBA and British passport.
