Name in Surrender Certificate

Hi,

I have surrendered my indian passport and received the surrender certificate today. I am bit confused about the name mentioned in the certificate. The names appear as below

NAME- My Firstname + Surname ( My Father Name)
Father Name - My Father name


This is how the name look in the surrender certificate ? Should the name part should have only my name or it included both my name and my father name ?

Appreciated if someone clarify me as i am about to visit birmingham tomorrow for OCI and dont want the things to get mess.
