Tier 2 ICT to Tier 2 Dependent in-country switch

hi,

i'm currently on Tier 2 ICT (valid till 2016). I would like to switch in-country to Tier 2 General Dependent.

filled the online application on gov.uk.

at the end of the application, it asked me to submit just 4 documents
1. passport
2. photos
3. marriage certificate
4. bank statements to show atleast holding 650£ pounds for last 90days as maintenance

my queries
1. can some one share the address to which the above documents to be posted
2. do we need to submit any other documents, or is it sufficient to submit above 4 documents
3. for last 90 days, in any one account i dont have £650. while filling application, i had added two bank statements (current, flexi-saver). at any point in time for last 90days, if i sum-up both accounts i would definitely have more than £1000. is this ok?
4. i didn't inform to my current employer about my application switch to tier 2 general dependent. is this ok?
