Tier 2 ICT Dependent - Name change

Hi There,

I am currently working in UK as a Tier 2 ICT Long term migrant and my wife has traveled as a T2 ICT dependent.
I would like to apply for a name change in her passport from her maiden surname to my surname.

1. Do I need to apply for a new Visa?
2. If yes, what are the timelines as I am afraid my Visa is expiring soon.
3. Can I change her passport and request the Visa extension with new name?
