evidence for absences for annual leaves + date of biometrics

1-I (like everyone) have had a few absences from the UK. Averaging 30 days an year for the last five years. The application says some evidence has to be provided that this was part of annual leave. Can either be letter from the employer or a self certification. Can someone share whether just doing a self letter is good enough an edit isn't frowned upon?
2-The application asked me date of my last biometrics ! I have NO idea of when this was. Can I leave that blank?

1) Have you requested your HR for holiday requests and you got approvals through email ( if thats the case you can print those email thread as evidence from your office outlook and stamped and signed by your HR or your reporting body at work place. and secondly , your in and out stampings while going and entering uk from your country for holidays.
( this is what i would suggest and you can take a second opinion from senior people in this forum.

2) you can say that you cannot remember ( or it would suffice if you can give month and year). I think if you applied by person at PEO, then it will be the same day your appointment was.

I hope this info helps. As i said , you wait for a seniors advice.
