Renewal of Annual Alloc granted but shows 0 in Lic Summary

Hi there Please see below the confirmation of Annual Allocation, 1 Cos requested in March 2015 and was Granted on 28 March 2015 via Email Confirmation to Level 1 User.

But now when we were issuing an Unrestricted Cos to our Existing Employee for Extension / Leave to Remain Application Under Tier 2 Visa at the point of assigning Cos sms says , there is Zero or insufficient Cos available to Assign

In License summary it says
Granted: Zero
Available : Zero

1- Please advice what could be the reason that the cos which was granted is not showing or available to assign the cos .?
2- Our Employee's visa is Expiring Next month Mid July and what will be best course of action .?
3- Should we contact sms or UKBA to find out .? if yes the best contact email address or Number.?
4-Can we apply for Allocation Increase how long would it take to get the permission.
