Student Dependent to HSMP

Hi
I've been going through posts here and they are very informative
I just wanted to see if someone could clear a few things for me

I am on student visa in the UK with my wife on student dependent. She is willing to apply for HSMP as she can claim 80 points.
My question is, if she gets HSMP, can she apply from being here or she has to go back Pakistan (Where I am originally from) to apply for EC?

Second, She doesn't have the birth certificate but she has the provisional licence. Is it ok if she sends provisional licence and notary copy of passport as the proof of age?

Third, She has a letter from University of Karachi in Pakistan stating that the medium of instruction there is English and she has done Masters from the uni. Does she still have to go for ielts or that letter would do?

plus, any idea what documents are required for EC in Pak?
