Question M4 and M5 on Tier 1, 2 or 5 dependant form

My wife is applying for a visa extension using Tier 1, 2 or 5 dependant application form. I'm naturalised but used to be under Tier 2 visa. I'm unsure how she should answer the following questions:

M4; Has the PBS main applicant received notification that their application has been
successful? Should the answer be yes even though I'm not be applying for any visa?

M5; Under which category of the Points Based System has the main applicant applied? Again, I've been naturalised but there's no option for that, so should she tick Tier 2 as this used to be my visa category?

Supporting documents - There's no clear requirements on what is needed, however in the last page of the application form it mentions utility bills, bank statements and council tax bills. Are these specifically for herself (ie in her own name) for the duration of her stay in UK, or are they looking for Evidence of cohabitation which is more strict (i.e on both our names over the past 2 years, from 3 different sources, etc).

Can anyone guide be to the queries below please? I also just realised the application form has been revised and now only includes Tier 1 and Tier 5 dependents. Tier 2 has been taken out completely?? Which form can we use now?
