Hi Guys,

I am UK citizen and planning to bring my mother and two brothers in Ireland under Eu directive 2004/38/EC (Freedom of Movement).
I am currently in UK. I am on first stage and am planning to apply for their Short Stay (C)- Single Entry visa. I will accompany them when entering Ireland after they get initial 3 months visit visa. I am filling online application form. I have below queries while filling application form.

1) What is the reason for travel? Family member of EU/EEA/Swiss cit. (other)... is that ok to choose ?
2)Purpose of Travel:............... family member of an EU citizen planning to exercise
free movement rights...... is that ok to write ?
3)Contact details for Contact / Host in Ireland. If you have no personal contact/host please give accommodation name and address.
What should I write here? As I am not in Ireland, I can't be host and I don't have address in Ireland yet. Should I write any hotel address here?
4) Is the contact/host in Ireland personally known to you (e.g: family / Friends)
What should I write here? As I will accompany them at time of arrival, so can I be their host/contact? or should I leave blank?

I am few additionak queries for attaching supporting documents. Please guide me
1) to prove financial dependency, I am going to attach bank statements, money transfer receipts. Are these documents enough? DO I need to provide any physical dependency too?
2) membership of the household: What documents should I provide to prove that? I could not understand thsi requirement. Please guide me
3) proof that the EU/EEA/Swiss citizen is exercising free movement rights in Ireland e.g. proof that the EU/ EEA/Swiss citizen already resides in the State or a declaration or statement of confirmation that the EU/EEA/Swiss citizen will be exercising those rights at the time of the applicant family member’s arrival in Ireland - this is so that the visa officer can ascertain that the applicant family member will be residing in the State together with the citizen concerned.

As I am not in Ireland already and will be accompanying them, what kind of declaration/statement should I provide. Could anyone please share me sample template for this?
4) Could any share sample cover letter template and documents complete check list?

You should find some of these questions are optional & do not need to be answered in the context of an EU free movement visa application.

There is also no legal requirement that you submit:

• bank statements
  pay slips
  letters from your present or future employer or school
  letters of reference
  proof that you will return at the end of the trip
  airline tickets
  confirmed hotel bookings
  references or guarantees from people in the destination country

Is there any who who applied Ireland Short Stay Visa C for dependent family members (Mother , Brother etc) before moving of EU citizen to Ireland himself? I mean accompanying Eu Citizen option.

Also Does anyone a template for declaraion statemnet that I will apply for excercise freedom of rights at the time of arrival of dependents family members?
