Short visit visa application form question

I am filling online visit visa application for my mother
Please help me fill following question in online form

1) Is this your first passport? Yes/No
Not first passport the but last one expies more than 10 years whats should I say above yes / no

2) Have you made an application to the Home Office to remain in the UK in the last 10 years?
In this question does application to remain in UK include any family visit applications as well

3) Have you been refused a visa for any country including the UK in the last 10 years?
My Parent had made family visit visa application 3 years back the first one rejected on lack of financial documents then we made 2nd application after two months which is accepted
should I mentioned 1st rejected application in above answer or not.
