Affidavit for different surname

All,

Excuse me if I am shouting in wrong forum or asking dumb question.

I have a slight mismatch in my surname in certain documents which I am planning to use for my HSMP application

Indian Passport and most finanical documents = xxxxxx Sharma
Bachleors Degree certifcette = xxxxxx Kumar
Indian Driving Licence(Age Proof) = xxxxx Kumar Sharma

Please can you tell me if I get an affidavit stating that all these names belong to same person, will that be enough for the case. If yes where can I get such an affidavit made in london as I will be submitting my application within UK.

If not, whats the alternate solution.

Also, Do I need to send notarized passport pages also with my application or not. If yes, where can I get them notarized.
