Surrendring Indian passport

Hi There,

I would be grateful if you could kindly answer the queries that I have regarding surrendering Indian Passport application.

1. Do we need to stick the photo in the application form - in the box where it says "Photograph 50*50" ?
2. What should we write in the 'Place of Issue' under British Passport details section? The place of issue isn't mentioned anywhere in the British passport :? .
3. For a Minors declaration form,I understand that father/mother can sign on behalf of child but in the name section where it says "I----hereby declare that....", whose name should be provided? Should it be the child's name or the parent's name who is signing?

1. yes, stick the photograph on the application. Also have one extra photograph to be handed over at the counter.

2. I've mentioned as United Kingdom and the application is in progress. So check with others for any other suggestions if not you can mention the same.

Thanks Alien.UK for helping me out, I just thought I will update this post for the benefit of anyone who would be in similar situation.

1. I took gluestick with me to the VFS office to stick photo if I was asked to, the person who issues the token at the centre, checked my application, stuck one photo in box leaving a small space for me to sign in! and stapled one photo with the application.
I had taken application for my husband and son as well, so, at the counter I was asked to sign under my son photo but luckily they didn't ask for any signature on my husbands application, under his photograph.

Following is the response i received from Information Service Desk,VF Services (UK) Limited info.inuk@vfshelpline.com

2.Please note that if the location of the issuing passport office (London, Liverpool, Peterborough etc.) is not known then IPS / UKPA / FCO are acceptable as the place of issue.

3. You need to mention applicants name and parents can sign in the declaration form.

In the Decalration form, I mentioned my sons name in the "I----hereby declare that....", and I signed it, writing applicants mother below it.
