PLEASE HELP!!! Child Settlement Visa

Hi,

I have gone through most of the posts here and can't find a scenario like mine. Request you to please help at the earliest as I have already the visa appointment.

I am currently a British Citizen after getting Naturalisation by Mar'16 i.e. after ILR which was obtained by
working in UK under Tier-2 ICT for 5 years. My wife got a Settlement Visa valid from Jan'15 to Oct'17 (33 months) while I was in ILR status. We submitted VAF4A online form & Appendix 2 with necessary documents.

Now, my baby is born in India by Aug'15. Since my wife already has a valid visa, I am applying only for my baby as CHILD dependent of a British Citizen. I need help with 2 questions,

1. Should I complete Appendix 1 or 2? Because I am British Citizen & my wife is having Settlement Visa, I am not sure which of the following I fit into.

You should complete Appendix 1 if you wish to come to the UK as:
The child of settled parents or with one parent who has limited leave as a partner or parent granted under the Rules in force before
9 July 2012

You should complete Appendix 2 if you wish to come to the UK as:
The child of a parent who is applying for entry clearance as a partner or the child who is applying to join their parent who is already in the UK and has been granted limited leave as partner granted under the Immigration Rules in force on 09 July 2012;

2. While submitting the application form online, I wasn't charged Health Surcharge. I should 0 INR because child dependent under 16. Is this correct (or) will they ask me to pay on the day of appointment?
