how to apply for family permit?

Hi i am in UK with my Lithuanian wife i have already applied for EEA2 which is pending. Now due to a close family member's death i have to leave UK , i want to ask how can i get my passport from Home Office to travel to my country and after that i will apply for a family permit from there which documents i need to provide for that?
i have UK marriage certificate, my sons birth certificate with my name on it. my wife and son will travel with me as well.
