URGENT - Extension tomorrow - some questions - panicking

Hi guys,

I am finalising my extension which is due tomorrow. I hv some questions, please answer asap. my life depends on this.

1) CT600 the HO expects and the
"Company - Short Tax Return Form CT600 (short) (2008) version 2 (the one my accountant gave me)" are same or not ?


2) what docs I need to sign ?
payslips
FPS
anything else ?
and should I say I confirm the above to be correct" and and sign on every FPS ?
