Returning back, how to get IHS Refund

Hi,
I had got T2 General visa for 5 years in Jan 2016, now returning back to India due to personal reasons.
I had paid IHS Surcharge for three applicants including me (i.e. £3000). How should I ask for a refund on my return, what's the process. Can any one please advice?

If my CoS & Visa get cancelled, should I not get a refund? As I will not be staying in the UK.
