EEA PR form - date first entered UK

Hi, i would like to add a similar question as an OP.
In the form there is a question in section 5A, date you first entered UK. I started living in UK in October 2006, but had been on 2-3 months holidays in UK before that, when i was studying and working. Do i enter the first ever visit to UK, which i think was in 2000? Do i have to prove this somehow?

When you answered Joszka, that he could use any 5 years during that 14 years of living here, where do you state which period they should look into? The form is just so unclear!
