Sworn Affidavit for child passport - online appointment quer

Hi there,
Can you please help with below query,

As per CGI Birmingham's site online appointment is required for service 'Sworn Affidavit for Issuance of Passport to a Minor Child Residing in Indi'

I tried to book one via below link but 'OCI Registration Number' is listed as one of the mandatory fields. Any idea what to provide here, I'm still on Indian passport. Is online appointment really required for this or can I go directly to CGI office and get a token there? I did try ringing their office but the number is always busy.

Just for future reference...

You can use your passport number as 'OCI Reg number' while booking the online appointment.

- Visit the CGI office 15mins before your appointment, below given are the docs I submitted
- 2 passport size photos (one for this form - http://cgibirmingham.org/form_files/Form_Pdf_66.pdf), they will give another form at the counter which you need to fill in.
- Passport copy (just front 2 and last 2 pages)
- Biometric (ILR) card copy
- Driving license copy as address proof
- Child birth certificate copy

Once the application is submitted wait for an hour or so and they will give you the affidavit there itself.
