Past Five Year Address History

While filling in the form under section 1.17 which is to provide Past Five years history, I have a question:

Initially When I first came I stayed for Nine months in a house (01/01/2011-09/09/2011)and then I left the house and gone to India.
Came back on 05/10/2011 and stayed at my friends house to 2 days.
Than I got a house which unfortunately I had to leave with in 10 days because of some issues in the house.
And then again I came back to my friends house and searching for new home and stayed for 10 more days there and finally I moved in to a house and stayed there for 2.5 years.

So I have a question how shall I fill all the above address:
- 01/01/2011-09/09/2011
- then I was in India (10/09/2011 - 05/10/2011) Do I need to fill Indian address?
- then from 06/10/2011 - 07/10/2011 in my friends house
- then from 08/10/2011 - 17/10/2011 in another house
- then from 18/10/2011 - 25/10/2011 again back to my friends house
- finally from 26/10/2011 --till date is one address

So how should I fill the above address or I can just say the first and last and skip the Indian and other 25 days address?

what I do is, I check my credit score report, and use the addresses/dates I see there..
