Indian Visa / OCI card doubt

Hi All,

I have recently got the Irish Passport through Naturalisation and now I am planning to apply for OCI Card and Indian Visa together (as OCI Card takes 3 months ) , but i do have below doubts --

1) What type of visa do I need ?

Do i need to apply the visa online(EVisa) from https://indianvisaonline.gov.in/visa/info1.jsp or shall i apply with Regular Visa Application from https://indianvisaonline.gov.in/visa/info1.jsp.

2) If regular visa then what type of Visa I need ?
Tourist Visa or Entry Visa

3) Also do i need to report to any police station once i reach India on any Visa even though my OCI application will already be in progress

4) OCI Card --

Do I need to get U sticker on my Irish passport , because it is bit confusing on Indian Embassy website , initially they are saying that "‘U’ Visa sticker has been stopped since September 2015 as the new OCI card has life time visa." and then later on on the same page they are mentioning that "OCI card holders travelling to India are advised to carry the Passport containing U Visa Sticker and OCI card to avoid problems at Immigration Check Points in India."

So do i need U sticker on my Irish Passport of not ?

5) Also i have to give up my Indian citizenship by submitting the passport so does the below RENUNCIATION_FORM is enough ?
