EEA(PR) Form help

Hello,

I've started filling the EEA(PR) form yesterday and I got stuck on some of the sections and how to correctly fill them. If anyone could help clarify some of my confusion I will be extremely grateful :)

First, some background:
* Both me and my wife are EEA nationals (Romanians) and have been in the UK since 2010. I'm the one that applies and want to include my wife on the application.
* During our stay in the UK I was/am the only one that worked. My wife did not work as she was full time mother (our daughter was born here and we want to register her British citizenship - hence our PR application).
* Also, from what I read on the forum, we do not need to include our daughter in this application.

Now, my confusion:
(A) I've included my wife on Section 1 - Subsection C (Family members included in your application) and I've left blank point 1.29 Relationship to sponsor. Is that correct? No one is her sponsor and Section 2 will not be filled as part of our application.

(B) In Section 3 (About your application) point 3.2 (Family members and extended family members). Do I need to select an option from there for my wife? By reading the options it doesn't look like as, again, there is no sponsor in our application.

(C) Section 5 looks like needs to be filled for both me and my wife. But I'm not sure what evidence should I include for my wife, if any... Will address evidence be enough as she did not work, did not study, was not a jobseeker nor was she receiving any benefits or tax credits?

(D) Section 9 - Subsection B - I've had an Accession worker ('purple') card but that has been replaced by a Registration Card ('blue card'). I do not have the details of the purple card any-more... How do I fill this subsection? Do I need to call the HO and get the details of my purple card (assuming that they can provide them)?

(E) Section 11 - this should not be filled if my assumption at point (B) above is correct.

Your daughter does not need PR; as she was born in the UK she has an entitlement to register as a British citizen once either parent acquires PR.

Has your wife had comprehensive sickness insurance for the entire period she has been here?

Your wife has to prove she was in the UK during the qualifying period you choose, so she needs electricity/gas/council tax bills, letters, bank statements etc with her name and address on

No, she had no comprehensive sickness insurance. I did not knew about it until you mentioned it... would making one now help?

Yes, we should be able to provide these for the duration of her stay.

Yes, we have all of these

Thanks for all your answers. Anyone knows anything about the other points? Will it be safer to apply only for me and do an application for my wife later? As I mentioned, the PR is mostly for our daughter's registration as a British citizen.
