EEA + non-EEA permanent residence application

Hi
I am non EEA national and my husband is EEA national, we are going to apply for PR.
we filled the EEA PR form (applicant and family member)
is it enough or I should fill a separate form EAA FM?
And in form EEA PR, should I fill section 11 or not as my husband is the applicant (and main sponsor)
?
I am a bit confused ..pls if some one could help me?
And does receiving income based JSA will affect our application, ? Which is already going to stop on 06/sep/2016?
