getting married before entering UK on fiance visa?

Back in April 2016, I was granted a visa to join my fiance in the UK. I had intended to travel in May, but I was unable to due to medical reasons (complications with pregnancy). My fiance is now in the US with me (for the birth of our baby), and we are considering marrying in the US before we return to the UK. My question is, does this change my visa status? Can I still enter on the same visa, which is valid until October, if we are already married?

Also, we've applied for a passport for our baby. Does anybody know if we need to do anything further to bring her with us?

You'll have to reapply for a Spouse visa.

What nationality is child?

Our child is British and American.

Alternatively, as my visa is expiring soon, is there any option to extend the visa to give us more time to marry in the UK?
