Format of parents name

Dear all

I am a bit confused as to how to write my parents name in the naturalisation form. In what order am i supposed to write the first name and the last name and my mum's maiden name?

Assuming my dad is John Smith and my mum was known as Jane Doe until she got married to my dad and is now known as Jane Smith, i am thinking of writing:

Father's full name:
JOHN SMITH

Mother's full name and maiden name:
JANE SMITH
DOE

Is it correct?

In the passport's application, we are asked to write the surname first so that got me confused.

Now on the passport application, for the name of the mother, do i write simply "SMITH JANE" and omit the maiden name? The maiden name does not seem to be asked but i might have misunderstood.

Bonus question 1. I know what year my parents got married but i don't know the day or the month. In the passport application, can i just fill in the year and leave the day and the month blank?

Bonus question 2. My father recently passed away. Am i supposed to indicate it somewhere in either of those forms?
