Help with PR form for European living and working in UK

Has anyone here filled in the PR form (85 pages ) for their PR after 5 years of residency in UK ?
Been in UK since 2002 , am a bit confused as to which years I should be including in my application form the first 5 ones or the last 5 years ? And what are the documents required for each year to submit for it ? I know they want at least 2 documents per year but any suggestions ? This application is doing my headin !

Many people have. Although the current monster form is not a mandatory legal requirement; there are earlier, simpler forms.

Its not all about the last 5 years.
More importantly when have you been exercising treaty rights?
And do you have rock solid evidence?

If a student did you have CSI?
If A8 did you have WRS?
