ILR Eligibility Date on Tier 2 General Visa

Dear Friends

I have never changed my employer and has been working with the same employer for the past 5 years approx.

First Visa
Visa Type :Tier 2 (General)
Issue Date on BRP: 28/11/2011
Expiry Date on BRP : 24/10/2014


Second Visa
Visa Type: Tier 2 (General)
Issue Date on BRP: 17/10/2014
Expiry Date on BRP: 06/11/2017

With reference to above visa dates, I would request you to please answer my below queries

1 Can I apply for my ILR at PEO(same day service) less 28 days before my qualifying period ie: 31/10/2016, whereas my full 5 year period ends on 28/11/2016

2 UKVI's current form in use for ILR application for my category is SET(O)/version 04/2016. What should I answer to the question on page 18 which reads as

Number of hours this salary is based on?

Rate of pay for this SOC code stated in the codes of practice (hourly rate or annual salary)?

However, my total weekly hours is 40 hours/week and my previous extension was based on SOC Code (3131)

3 I have been on holidays out of the UK while I was holding Tier 2 General visa and I have stayed more than 90 days out of the UK. However, my leave was approved by my employer and I use to work online as agreed with my sponsor and there has been no deduction in my pay and full NI and Tax has been paid for that duration.

Is this going to be a problem, How do I prove this.
