Self employment proof

Hi. Can someone please advise on my following question regarding details of employment and self employment for the required 10 years employment?my husband worked for the last 10 years all througout ; he worked as an employee for one year from 2006-2007 and has P60 and P45 when left; then self employed from 2007- 2013. Where does he add the self employment details ( on the same list with employment? ) and what proof does he need to provide for that? Is this proof - his last self assessment statement of account 2012- 2013 ( he stopped being self employed in Dec 2012)? would this be enough? Or proof for every year of his 6 of self employment?
He worked from 2013 to present as employed again so has P60s and P45s when changed.

The info regarding his self employment proof is crucial as we really are not sure. Please advise us. I have asked this somewhere else as well and no reply so apologies for repeating.
We really appreciate and trust this forum so can someone please give advise.

Enter all employment and selfemployment details in same list.

Simply include SAs and P60s for the relevant period to help show all obligations vis a vis HMRC have been met.
