EEA registration certificate and name change

Hi everyone!
I really hope, that someone can help me as I am getting really confused.
I have EEA registration certificate for Qualified person (EEA QP), it is blue paper one, not the biometric, and my last name has changed.
So i tryed to look up on GOV.uk how to change it and even phoned EEA line.
The answer on the phone is that I need to reapply, and that there is option for replacement card.
Now, on Gov.uk it says to reapply if card is stolen or damaged or lost.
I do not have issue with paying 65 quid, but I do not see why i need to fill this massive application all over again, is there any smaller form/other option?
please help!!!!
