EEA child born in the UK from EEA parent

Dear all,

My child was born after my husband who is portuguese acquired his PR. But he only applied for his permanent residency card in 2015. She was born in 2009 and he acquired his PR 2 months before she was born. All the time working.

How can my child apply for British Passport and the form we need to use?

Passport form - from Post Office.

Child Rules for BC application

Dear All,

I need help with the following situation.

Father from Portugal . Came to UK in 07/04 acquired five years working for the same employer on 07/09. First child born in 09/09 and second 08/13. Wanting to apply straight to British Passport.As evidence the father has HMRC employment History and Employer letter only.

Questions :

1) Is it correct to apply to British Passport straight?
2) If yes what are the steps?
3) Are the evidences sufficient?
4) If this is not the right path what is the best?
