Extension aplication form; help required

Can you please help with these questions. i am applying with 4 dependants so cant risk of rejection due to silly mistakes in form completion.
Section L of application form:
1): L1: Do you wish us to take your entry into the UK as your specified date.
I applied from within UK so i believe i must say NO. Am i right?
2): L5: stating you must confirm you have supplied a printout from companies house of the company filing history page and printout of current appointment report.
I need to provide initial Company house registeration copy and current appointment report?

3): Section N3: I did not join existing business so does not apply on me? I must not tick any of these?
4): What is the meaning of job number in table 3b2. Does it mean job1 and job 2?
5): Section 4: English language. I provided degree taught in english for initial application. Should i simply tick evidence previously provided or attach degree again. some people on forum mention to attach degree again.
6): I renewed my paasposrt recently from London. Do i need to state it in C4 place of issue of passport as london or i simply put name of issueing country.

It should be as stated in your passport which I guess is London
