Tier 2 Sponsorship License Small to Medium change

Me and my friend started a company few years back and now we are planning to apply for Tier 2 sponsorship License. When looking into the fee for application, we are in bit confusion. My query here is

1. Can we apply license under small company paying the fee of £536 and change it to medium company in future?
2. What will be the fee if we want this change?
3. Is there a time limit to make this change?
