Naturalisation and British passport

Hello Guys

I am applying for naturalisation and British passport via NCS for my whole family (2 adults + 2 children). I was on Tier 1 category (employed + self employed)
My question is in Application form : Section 3.3 says
1. "Do you have any Civil judgement or Civil Penalty against you under immigration Acts ".
2. "My Accountant will be my professional Reference, Can I use his Reference for both naturalisation and British passport application.
3. "Employment History ". I am currently employed but I was self employed for few years. So do I need to fill those details as the form.
4. Just to make sure I am using the correct application form. For Adults it's form AN (January 2016 version) and child form MN1 April 2016 version)
5. I have got FPN in January 2014 for motorcycle which I declared in ILR. Do I need to declare it again"

For question 1. I have got a Court Order for Possession for eviction (Accelerated procedure, Assured Shorthold tenancy) last month. My Landlord has taken me to court to claim his
property back. I have paid rent regularly and never defaulted.
Do I have to declare this in my form ?
