what hour rate should we fill in the form

hi
i have a employee working 40 hour per week and get pay £9 per hour (this one showed in payslip)
when i fill in the form and table about hour rates
should i take (40x9)/30 = 12 in to the form or should i put £9

The form says hourly rate, that is what you put in the table which is £9 !!
