HSMP Document List

Hi All

I am new in this group. Next week I am going to apply for HSMP. For
List of document I have prepare.

for Educational Qualification:
1) MCA Original Degree Certificate.
2) MCA Transcript
3) Medium of Instruction Letter. (I know this is not required but from
my University I got this letter so i am submitting)

For Earning Power:
1) Tax paper (Form 16 of April 06-07 and Tax return form)
2) Jan to Dec 2007 (12 months) Bank statements
3) Jan to Dec 2007 Salary slips sealed and signed by HR.
4) Letter from HR

For Age:
1) Birth Certificate
2) Original Driving License

for English Proficiency:
1) BCom Degree Certificate
2) Medium of Instruction letter


for English Proficiency my collage will issue BCom Transcript after 10
days. As per my knowledge Transcript is Optional. In Bcom Transcript
it is mention that medium of instruction in English. My QUESTION is
should i wait for 10 days or I can apply without waiting this?. and
let me know these all documents are enough or i am forgetting
something.And Please someone have Covering Letter sample please
provide me at bmsinfotech@yahoo.com.
