AN form, how to fill, what to attach

I have 3 questions:
1)I received PR card 2 months ago,I m EEA member, on my PR application I asked about confirmation the DATE WHEN I ACQUIRED PERMANENT RESIDENCE IN UK. but received only card, nobody confirmed the date for me.
I need to confirm the date because I wish to apply about british citizenship immediately and don’t want wait 12 months from the date when my card was issued. I m nearly 11 years in UK. So I acquired treaty rights a couple of years ago, following 5 years residence in accordance with EEA regulations, not on the date when I get my card 16 november 2016.
Date of issue on my first WRS certificate says 19 september 2006. So please clear it for me please Am I right, I acquired permanent residence in Uk from 5 years from these date? 19 september 2011? Plus one year if I want apply about citizenship. I need put date on my application about naturalisation.

2)I should apply based about what:

-APPLICATIONS MADE ON THE BASIS OF RESIDENCE IN THE UNITED
KINGDOM
Evidence of lawful residence during the 5 (or, if the applicant is married to or in civil
partnership to a British citizen, 3) years before the date of the application
• Your passports OR say why you are unable to provide one on page 24 and
supply
• Letters from employers, educational establishments or other Government
Departments indicating the applicant’s presence in the United Kingdom during
the relevant period

OR

-FOR APPLICANTS FROM SWITZERLAND OR THE EUROPEAN
ECONOMIC AREA
Evidence of Nationality
• Your valid passport or valid EEA national identity card as evidence of your
nationality.
Evidence that you are considered permanently resident in the UK
• A document certifying permanent residence or a permanent residence card
issued by the Home Office.

I would mention I m 11 years in UK, card was issued 2 months ago and I want apply immediately not wait 12 months from the date on the card, if I acquired rights couple of years before, so my 12 months passed already long time ago. If I m correct.

3) so clearly from what period I need to show evidence and what I need to have? last 4 years I been self employed. application guide not clearly says what I need to show.

Get a SAR or apply via NCS and NCS will check relevant date for you.
That is if kindly caseworker did not enclose a letter stating your date of acquisition.

Naturalisation is not a rerun of PR.

You have to prove residence in UK for past 5 years plus proof of physical presence in UK on the day 5 years before date of application.
And prove you are settled in UK.
And you have to provide proof of meeting tax/NI obligations for up to 10 years of employment in UK - or since you entered UK and worked (if less than 10 years).

I can see people sending council tax bills for each year, letter from employers, and dont think so if I have everything, but I have p60 from 2006 till 2013 and from 2013 till 2016 self assesment.

so if I send p60 and self assesments for last 10 years, plus RP card, b1 test and life in uk, of course fee, ID, will be enough? did I missed something?

No it won't be enough.

P60s only show you met tax obligations.

You have to prove residence in UK too and you have to do it in absence of stamps in passports.
That's why people send other official docs such as CT bills & etc.

if I dont have CT bill from the past? what is also accepted?

As per guidance.

For example, government or local government and/or quasi-official documents:
  • Tax documents;
    Utility bills;
    Driver's license;
    Voter registration/electoral roll;
    Insurance (house/car);
    Medical letters/registration with RHA;
    Doctor/Dentist/Optician letters;
& etc etc

Unless living in a cave you must have some of these.
