UK Tier 2 Visa Urgent Help

Hello Experts,
I got refusal decision letter on 12th this Month. I applied for UK Tier 2 General visa at VFS office, New Delhi.
My visa application is rejected on wrong reason (maintenance) despite I submitted relocation grant from sponsor to a value of more 2000 GBP., but in refusal decision letter they said they have not received such a letter. Additionally when I got refusal decision letter packet, Admin review notice form was missing and in decision letter , it was clearly written I have a right to send one admin-review notice form. Even though I found a admin-review notice form online but dont know to which address have to send? What shall I do in such a situation? Please help me.
