Child Settlement Visa-Healthcare Surcharge&Appendix&Other

Please could you help me here?

I am planning to apply for my child to join me and settle in the UK.

I am divorced with the legal custody of my child, who is only 16 months old. I consider that I have sole responsibility for my son because not only I having the legal custody, but I also pay for all the child cost and make decisions for him. During the time I am not with him, we do facetime every day. I have active involvement in all aspect of his life. Whilst child's father has no involvement at all , doesn't pay child support or visit the child. He lives in another city in China.

Child's father has Chinese nationality and lives in China. He doesn't have UK visa.

Myself, I had UK indefinitely leave to remain since 2008. I have become British citizen in November 2016 and that was after my baby was born, therefore, from what I understand, he's not entitled to the Right of Abode. I gave birth in China and came back after my maternity leave.

Here are my questions,

1. Is it the right application - Settlement - Child settlement of settled person in the UK

2. In my case, should I include Appendix 1 or 2 with the application?

3. If it's going to be appendix 1, for question 2.14 How much does your sponsor spend each month on
living costs? Shall I include rent and council tax in this calculation or everything for basic living (Rent, council tax, utilities, food cost.)

4. On the healthcare surcharge, I tried the online calculator, which the result says £0.00, is this right? I'd like to make sure that I am not missing anything and should be paying for it.

5. Sponsorship undertaking: form SU07, do I need to complete this form with child's application or this is just for adult relative?

If you are indeed the sole carer , then Paragraph 298 is engaged and child can apply for ILE. There is no NHS surcharge for that type of application.
