Tier2 Dependent for child born in UK -Applying visa in India

Hello Guru,
I need your help in applying tier-2 General dependent visa from India for my six months old child born in UK.
My child is born in UK six months back, we need you travel to India this month urgently. As I have no time to apply dependent visa through post for my child only in UK I am planning to apply it from India. Can you please help me out with my queries

1. As visa is only for my child, is the minimum balance maintenance is £630 for 90 days?
2. Is it okay if i show this in more than one bank account? I have 2 bank accounts ( one personal and 2nd is Joint) and in any date i have more that £630 but if you look into one bank account then balance is going less than £630.
3. Or as my employer is A rate sponsor, can I get a letter from them on funds availability if I require
4. Does my child need a TB X-ray? As she is 6 months old and stayed in UK for 6 months? Actually my Tier 2 general visa will expire by 21-8-2017 and my child application submission date is 24-Feb-2017. So my child will have a visa valid until 21-8-2017 which is less than 6 months. Does my child still require it? Please do let me know if I am missing anything else.

5. While applying from vfs site do i need to take the below one for my child?
Work - Tier 2 (General) visa - General Child, up to 3 years.

6.List of supporting Documents:
Child Passport
UK Birth Certificate
Father (Main applicant) and Mother's Passport & bio-metrics card photo copies.
Bank Statement or Employer letter on funds
marriage certificate
UK and India Address proof

7. Do we need send my original passport and bio-metrics along with the my child application?

4. no. her country of residence is already uk and she is not going to have lived in india for 6 months before she applies.

Regarding fund maintenance for dependent child,
1. Is it ok to show Indian bank account with minimum balance £630 for the last 90 days? Or we need to show only UK bank account statements.
Or
2. I didn't get proper answer from all, whether to show multiple UK banks account statements?

Is it okay if i show this in more than one bank account? I have 2 bank accounts ( one personal and 2nd is Joint) and in any date i have more that £630 but if you look into one bank account then balance is going less than £630.

2. If statement amount is less than 630£ than ignore this option

or you can get a letter from your sponsor if it is A rated company
