URGENT: English requirement for Tier 2 (General) visa

Hey all,
I applied for tier 2 general visa for a job in a NHS trust. I am a doctor from India, I have a masters in Anaesthetics from India. I have worked as a Anaesthetist in the same UK trust last 2 years in tier 5 visa. Also I had given IELTS at that time (validity expired now)
This time around my visa was refused on the ground of not providing a letter from UKNARIC. So i got 2 letters from UKNARIC regarding my degree. So my questions are

1) I had only provided my bachelor's degree MBBS, as i did not have the transcripts for my masters exam. Will that be a problem
2) UKNARIC has mentioned that my degree is
comparable to primary (first) UK qualification in medicine/veterinary/ dentistry
My doubt is does that mean its equivalent to bachelor's degree.
3)Does my wife also needs UKNARIC certification for her degrees, when she is applying for PBS dependant tier 2 visa. P.S - She is not into medicine, her background is microbiology and MBA (human resources)
4) Is there a time limit after which you can be eligible to reapply post tier 2 visa refusal.

I know these might be silly questions, but i need to be completely sure before i reapply, as i have already incurred lots of financial losses and frustrations.
