Biometric appointment documents question

Hi

I have to go back to New Zealand soon to attend my biometric appointment and post my application, this is after I submit my online application and pay the fees. Can someone who has recently done this tell me what they sent off? I understand it's your passport, a print out of the application and any supporting documents. Can anyone confirm if you still have to send an Appendix 5 form and 2 photos along with everything or have the requirements been updated now?
