Documents required for Tier 2 General Visa Extension

Dear all,

I will be applying for Tier 2 Extension this month. Could anyone please assist me with the following;

1. My passport had expired an year ago and got a new one. Do I need to submit my old passports while applying for extension?
2. I have 4 dependents ( wife and 3 kids). Do I need to submit my marriage certificate?
3. Do I need show the maintanance fund? Employer has mentioned in the CoS they will be showing the fund.

Please help me with the complete list of documents required for myself and my dependents ( wife and 3 kids).
