Change last name in Passport - EU Family member

Hi, I am from Cape Verde and I came to England about 2 years ago through my husband that is portuguese. I currently have a family member- Eu residence work permit card. My name in my passport and my documents in England are with my name before marriage. I will now update my name in the passport adding my husband's surname. I would like to have an advice if possible of how to change my name in my residence and what is the best process to change the name in all different government establishment. Many thanks
