Income requirements from 2 jobs for FLR M

Dear All,

I am going to apply FLR M next month and my sponsor is working at two places to exceed the required income requirements. While filling the FLR M form;

1) Do i have to put my sponsors two employers details and salary in section form 7.3 A in small columns or put in additional sheet provided in form?
2) Do i have to put my own salary details as well even though my sponsor's salary exceeds income requirements?
3) Do i need to also submit my sponsor's contract letter or just employment letter will be enough ?

Any suggestions would be highly appreciated. I am currently in Tier-4.
