PASSPORT RETURNED WITHOUT VISA BUT ALSO NO REJECTION

Hi,

I haven been applying for a visa for some people I am working with. It is to take a performance from Belgium to the UK. We had invitation letters from the festival and applied for STANDARD VISAS.
2 of the performers went on the same day, one had his passport returned on time with his visa approved. The other didn't hear back until I submitted an e-mail to ask what is happening, we then heard that his passport is on the way.
However upon receiving it he hasn't been able to locate any visa in his passport, but also no rejection notice or reason for rejection. The envelope had all his support material as well as his passport but nothing else.

Has anyone experienced this? Is this a rejection or some strange bureaucratic mistake?
