wrong COS in TIER 2 ICT application submitted..Help please

Need urgent help please.

I have submitted previous (old) cos number into my online visa extension application.
I have spoken with VISA immigration office and they advice me to submit a covering letter with new COS number..Can anyone please advice me how to write a covering letter in this case please.

My premium appointment to extend visa is in next week along with my dependent.

There is no prescribed letter format.
Keep it simple - put old COS by mistake- here is the correct one. They don't want a novel just the right information :D
