varying FLR(FP)-PRIVATE LIFE TO SET(LR)

I need help please.

I know what to do to vary my case from flr(fp) to set(lr) but I don't know the form to use for my dependant(wife) for variation or should I leave her on the previous form with the HO?

PLEASE ADVICE.
