Tier 2 Extension application - declaration on last page

Hi guys,

I've searched about this on the forum but I couldn't find any answer although I found someone's raised this before.

My question is, when you download the completed Tier 2 extension application, on the last page, Declaration section ends like below;

I confirm that:
I am the applicant

Sorry for being so daft, but I cannot figure out whether I have to sign next to it or write a statement declaring the accuracy of my information etc?

Any help is greatly appreciated!

When you fill the online form, declaration has already been done. So no deed to sign.
