Tier-2 Extension with same employer

Respected All,

I am on Tier-2 (SOC-2135, Job Title: Data Analyst with Pay band 5 £28,460) within an NHS trust (A-rated). My extension is due in Oct-17. My queries are:

1): Does my employer need to do RLMT if I am promoted to an other department in Band-6 under the same SOC 2135 with same job title Data Analyst but increased salary (£32,407)?

2): Does my employer need to pay £3,000 (Skills Development Surcharge) for my extension? (I came to this country in Oct-2009 to study, and I moved to Tier-2 in Feb-2014 from PSW, will qualify for ILR after Feb-2019. It means if my employer issues me COS for two years they will need to pay £2K in Surcharge). Any suggestion in this regards?

3): Any advice on submitting documents for extension?
