Need advice set(o)form after 6 years discretionary leave

Hi everyone,I need some help, i completed 6 years dlr ,I am filling the set (o) form at this moment.
1. In the section 5 , it ask about finances,do I need to fill in the questions after the 5.5 (about the public fund).Such as soc code, current rate of pay, also 5.6 5.7 5.8 5.9
2. In the section 6.1 the first day enter uk. Is it the date we got our first dlr,or the date we in got in the uk( I get in before I got my first dlr)
And also in 10.12 have you had your fingerprint taken as part of a previous uk immigration application made in the uk or abroad? Yes or no.
I confused , I did the fingerprint when I got the first and second dlr biometric card.
